Case Manager

Position Summary

Safro Staffing is partnering with a behavioral health and addiction treatment organization seeking an experienced Case Manager to assist clients in accessing resources, coordinating services, and supporting successful treatment outcomes. This position serves as a liaison between clients, treatment providers, community agencies, and support systems.

Responsibilities

• Coordinate client services and discharge planning.
• Assist with aftercare placement and recovery resources.
• Support clients with transportation, housing, employment, and community resources.
• Assist with disability, FMLA, and other documentation needs.
• Coordinate appointments and outside provider referrals.
• Maintain accurate case management documentation.
• Collaborate with clinical and medical staff.
• Advocate for client needs and treatment goals.
• Monitor client progress and barriers to success.

Qualifications

• High School Diploma required; Associate's or Bachelor's Degree preferred.
• Experience in behavioral health, substance use treatment, social services, or case management preferred.
• Strong organizational and communication skills.
• Ability to manage multiple priorities.
• Knowledge of community resources and recovery supports preferred.
